The applications are open from November 1st to 30th 2019.
Please follow this steps to apply:
STEP 1
Fill out this application form.
STEP 2
Send a message to
upg-cursos@med.uminho.pt with the following documents:
a) Copy of the medical license or registration certificate at “Ordem dos Médicos”;
b) Copy of the medical degree certificate, with the final classification;
c) Professional, scientific and academic CV, according to the template available
here.
STEP 3
Proceed with payment of the application fee (40,00 EUR)
a) Register at
UMinho Store(top left corner of the page), with your name and the email provided in this application form.
b) You will receive an email with a confirmation link. Click on the link.
c) Add a password to your account and repeat it.
d) Return to
UMinho Store and click on the button "Add to cart".
e) Click on "Checkout" and follow the remaining steps to complete your order.
Payment can be made with Credit Card (Visa and MasterCard) and ATM ("Multibanco"). Further details are available
here.
The application fee is non-refundable.
The correct indication of customer data during the payment, including a correct fiscal number, are the sole responsibility of the customer. UMinho is not responsible for any errors that may result from inaccurate information provided by the customer, particularly in the issuance of tax documents which are not susceptible to cancellation after issued. If you do not indicate the tax identification number you will be considered as final consumer (non-taxable person).
