FULL COURSE ::: April 20th to 24th, 2020 Complete theoretical-practical course FULLY BOOKED! Fees: - ENT Specialist: 1400,00 EUR - ENT Resident: 1300,00 EUR Includes: - Hands-on in a full-equiped Lab - Surgical Videos in pendrive - Coffee breaks and Lunches - Official Course Dinner and Group Photo - Access to Best Temporal Bone Dissection Contest 2. COMPLETE THEORETICAL COURSE ::: April 20th to 24th, 2020 All-days Theoretical Course Theoretical sessions are limited to 30 participants at once Fees: - ENT Specialist: 400,00 EUR - ENT Resident: 300,00 EUR Includes: - Coffee breaks and Lunches - Surgery video-sessions - Access to “Bring Your Case” Contest (only for residents) 3. BASIC THEORETICAL COURSE ::: April 20th and 21st, 2020 Lectures and Video Sessions - Monday and Tuesday Lectures and Video Sessions are limited to 30 participants at once Fees: - ENT Specialist: 250,00 EUR - ENT Resident: 150,00 EUR Includes: - Coffee breaks and Lunches - Surgery video-sessions 4. ADVANCED THEORETICAL COURSE ::: April 22nd to 24th, 2020 Lectures and Video Sessions - Wednesday, Thursday and Friday Lectures and Video Sessions are limited to 30 participants at once Fees: - ENT Specialist: 350,00 EUR - ENT Resident: 250,00 EUR Includes: - Coffee breaks and Lunches - Surgery video-sessions - Access to “Bring Your Case” Contest (only for residents) Special Discounts 1. EAONO MEMBERS EAONO members will have a 10% discount in the fee. 2. BRING YOUR CASE CONTEST All Residents, upon registering, may submit an abstract of a clinical case. The 4 best abstracts will be selected for discussion (12 minutes) in the last day (Friday morning). The selected Residents, based on their registration mode, will have a discount in the fee: - Full Course / Complete Theoretical: 200,00 EUR discount - Advanced Theoretical: 100,00 EUR discount Residents who enroll in the Basic Theoretical module are not eligible because of the calendar mismatch. Details for the payment This information will be sent to the accepted participants Registration fee payment policy After submit the registration form, you will receive an email with the payment details (this may take up to 72 hours). To complete the registration process, you should proceed with the payment and send the receipt to upg-cursos@med.uminho.pt within 5 days. The correct indication of customer data for the invoice/receipt, including a correct fiscal number, are the sole responsibility of the customer. UMinho is not responsible for any errors that may result from inaccurate information provided by the customer, particularly in the issuance of tax documents which are not susceptible to cancellation after issued. If you do not indicate the tax identification number you will be considered as final consumer (non-taxable person). Your registration will be effective after confirmation of the payment. If payment is not made in due time we will have to cancel the registration. Registration fee refund policy We cannot refund or return the fee, except in case of the course suffer any changes or it is cancelled.
